| Dancers will be selected by audition to become
part of DanceQuake – a new program of Get Up and Dance! Bay Area. Made possible
through the generosity of Ronald McDonald House Charities® of the Bay Area
and other sponsors, DanceQuake is designed to bring the joy and benefits of
dance, movement and fitness to school-aged children and families throughout
the Greater Bay Area. Get Up and Dance! Bay Area is made possible through
the generous support of the Northern California McDonald’s® Owner Operators.
The program was created by DuDell & Associates, Inc. and is executed
by Dancers’ Group with choreography by Micaya, Artistic Director of the San
Francisco Hip Hop Dance Fest. |
| In order to register for the Get Up and Dance! Bay Area auditions you must carefully review the following important information. In consideration of the opportunity to apply for an audition, you must agree and consent to the following conditions and understandings: |

I understand that the audition held Sunday, July 22, will be limited to 500 people and that it is possible that I may not get an audition. I also understand that if I make it through Round One, I will need to return that same afternoon for Round Two call backs; and if I am then selected as a finalist, I will be required to participate in preparation for final auditions the evening of Wednesday, August 1, 2007, as well as the finals on Saturday, August 4, 2007. |

I agree to and do release and hold harmless the
GAAF Co-op of Northern California McDonald's® Owner
Operators, RMHCBA, McDonald’s Corporation, its subsidiaries, related companies
and their franchisees, DuDell & Associates, Inc., Dasani / The Coca Cola
Company, Dancers' Group, and ODC Dance Commons, and all of their agents,
representatives and employees (the "Released Parties") from any claims that
may arise as a result of or in connection with my participation in the
audition. |

Participation in this program involves strenuous activity and should not be engaged in by any person who is not in excellent physical condition and health. As a condition to participation in this program you must voluntarily and knowingly agree to assume all risks related to your physical condition and health, and you must represent and warrant that to the best of your knowledge you have no health or other medical problem or concern that might place your health or well being at risk by participating in this program. |

I give permission to the Released Parties to use my likeness, photo or video for the purpose of media relations or promotions, or for any other purpose reasonably connected with Get Up and Dance! Bay Area or DanceQuake. |

I understand that if I should be chosen to advance further through the selection process I will be required to provide additional personal background information. I agree to provide accurate and truthful information as requested and also agree to authorize any of the Released Parties to perform background screening, including reference checks, as they deem appropriate. |

I understand and agree that if I am selected by the judging panel to be one of the DanceQuake dancers or a swing/alternate, I will be contractually required to attend rehearsals, meetings, costume fittings, communication training, media opportunities, performances and other appearances, and I agree to do so. |

I understand and agree that if I am selected to be one of the DanceQuake dancers or a swing/alternate, I must conduct myself at all times in a way that presents a positive image for the GAAF Co-op of Northern California McDonald's Owner Operators, RMHCBA, and all others connected with Get Up and Dance! Bay Area and DanceQuake. Accordingly, I agree to conduct myself in a manner that is at all times consistent with the highest ethical and moral standards. Also, I will not act in a manner that could shed a negative light on the DanceQuake dance team, the Get Up and Dance! Bay Area program or any of the Released Parties. |

I affirm that as of the date of the first audition (July 22, 2007), I will be 18 years or older. |

I understand that the judges' decision with respect to selection of DanceQuake dancers is final, and that program producers retain the right to select the team members, refuse or terminate participation by anyone for any reason, and make changes to the team, at their sole discretion at any time. |

I acknowledge that if I do not check in by my assigned call time on the date of the first audition (July 22, 2007), I will forfeit my opportunity to participate in the audition. |
